This role focuses on delivering high-quality physical therapy services based on physician orders, in a consultancy environment. The assignment involves travel to various counties including Independence, Sharp, Izard, White, Fulton, and Jackson.

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete a comprehensive evaluation using appropriate tools and procedures.
• Review patient records for diagnosis, medical history, and precautions.
• Establish measurable goals and develop a treatment plan to promote patient independence.
• Regularly reassess treatment effectiveness and patient progress.
• Communicate patient needs and progress to the treatment team and related parties.
• Consult with medical providers about patient evaluation, progress, and discharge.
• Recommend and assist in securing adaptive equipment for patient improvement.
• Maintain and submit required documentation in accordance with regulations.
• Supervise Physical Therapist Assistants and Aides in direct patient care.
• Participate in and conduct development programs and meetings as required.

Requirements of the Physical Therapist

• Current PT AR License
• Experience with HCHB, OASIS, and Home Health
