Physician for Skilled Nursing Facilities - Primary Care West Haven - Waterford

Job Opportunity

Physicians Alliance of Connecticut (PACT, LLC) has an Internal Medical Group with an opportunity for a physician to work in skilled nursing facilities in the Waterford area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The position would be full time working 40 hours per week.

Under indirect supervision, provides and manages health care for a specified patient population.

Essential functions include:

• Providing and managing direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Engaging patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documenting individualized care plans and care coordination in the patient chart.
• Analyzing patients medical history, medication allergies, physical condition and examination to determine best care.
• Diagnosing medical conditions.
• Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
• Referring patients to specialists or to internal medicine as appropriate.
• Directing and coordinating the patient care activities of nursing and support staff as required.
• Respecting and promoting patient rights and privacy.
• Responding appropriately to emergency situations.
• Participating in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
• Participating in specified health promotion, education and/or prevention programs.
• Participating in population health activities as directed by the practice.
• Following established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.

Skills and knowledge required include:

• Knowledge of legal and ethical standards for the delivery of medical care.
• Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
• Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
• Demonstrated skill in interviewing and patient coaching.
• EHR documentation as it pertains to patients and populations of patients.

Education and experience required:

• Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
• Medical License, Board certification, Federal DEA certification
