Job Summary LPN: Provides each resident with professional nursing care, assessment, and evaluations within scope of practice. Required Education, License/Certification, or Work Experience LPN – Board of Nursing license in state of practice upon hire. RN – Board of Nursing license in state of practice upon hire. Work Schedule 6:30pm–6:30am: E/3rd Wknd/Hol; 72 hours/2weeks. A Brief Overview Delivers nursing care by developing a nursing diagnosis; establishing nursing care objectives; defining nursing interventions; providing physical, emotional, psychological, and spiritual support to residents, families, and friends to understand residents conditions, medications, and self‑care skills; noting changing conditions and needs; re‑evaluating care objectives. What You Will Do Provides resident care by reviewing physician prescriptions; communicating nursing care plan and pertinent information to team; coordinating with other providers; providing support; conferring with physicians. Develops nursing care plans by obtaining medical histories; examining residents; documenting physical condition, psychological status, social factors; conferring with family; establishing diagnosis, objectives, interventions, schedules, and timetables. Teaches healthcare requirements by establishing care goals; instructing residents, family, and friends; answering questions; providing information and resources. Evaluates resident care by adhering to therapeutic standards; measuring outcomes; reviewing records; noting changes; re‑evaluating objectives; modifying the plan. Maintains safe, secure, and healthy environment by adhering to infection‑control, medication administration, storage, and controlled substance protocols; following organizational standards, policies, and legal regulations; alerting others of concerns. Keeps resident care equipment operating by following instructions, troubleshooting, maintaining supplies, performing preventive maintenance, and requesting repairs. Keeps resident care supplies ready by checking stock, anticipating needs, placing and expediting orders, verifying receipt, and stocking items. Coordinates admissions, discharges, and transfers to and from the facility. Directs, plans, and coordinates workflow of nursing assistants and other nursing personnel. Maintains competence in nursing capabilities as outlined by leadership. Essential Qualifications Must be able to work specified hours. Must be able to perform essential job functions satisfactorily, including visual acuity adequate for duties, and effective communication, hearing, understanding and distinguishing speech and sounds.
