Reporting Relationship: Martin Youth Center CoordinatorSupervises: YES: NO: XFLSA STATUS: Non-exempt-level, employment-at-will positionPURPOSE: Directly responsible for: direct care services to clients as assigned. The service will always be a part of a treatment plan authorized by a Qualified Mental Health Professional. Adhere to the agency mission, policies and procedures.ESSENTIAL JOB FUNCTIONS: \Provide individual attendant care as neededParticipate as assigned in planning and in implementation of activities.Provide psychosocial rehabilitation services as assigned.To work with clients to achieve their personal objectives and goals as specified in their treatment plan.Keep accurate records as recorded.Regular and predictable attendance and punctuality..ADDITIONAL RESPONSIBILITIESParticipate as assigned in planning and in implementation of activities.Participate in all required meetings.Assist in other programs or departments in the Center as needed to meet company needs.Provide care and maintenance of Center equipment and space.Assist with van driving for clients and have certified van training, if assigned..Other duties as assigned.The statements contained herein describe the scope of the responsibility and essential functions of this position, but should not be considered to be an all-inclusive listing of work requirements. Time spent away from the Center would include traveling. In exercising these functions the incumbent should be able to exert up to 50 lbs. of force occasionally, and/or up to 25 lbs. of force frequently, and/or a negligible amount of force constantly to move objects.CONFIDENTAL: All employees of CKMHC are expected to maintain the highest level of confidentiality.LOCATION: Salina. Maybe assigned to an outreach area.REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:High School Diploma or equivalent requiredPreferred - related human services experienceCompletion of required agency and state training.20 years of age and 3 years older than member under age 19Clearance of KBI Adult and Child RegistryHave an understanding of the characteristics of mental illness.Have a working knowledge of the Community Based Services Program as well as the Community Support ProgramHave the ability to effectively work in a team.Have the ability to effectively relate to clients, their families and the public in a positive way.Have knowledge of local human service systems.Have knowledge of current policies and regulations governing eligibility for clients.Have an ability to communicate clearly and work effectively with members of all socio-economic groups.Produce documents written with clearly organized thoughts using proper sentence construction, punctuation, and grammar.Have the ability to learn job related material primarily through oral instruction and observation.
