Support Patients in Maintaining Daily Function at Home What You’ll Be Doing Implementation of Therapy Plans Carry out treatment plans developed by the supervising Occupational Therapist. Support patients in practicing functional daily living skills. Hands-On Functional Support Assist with self‑care tasks such as dressing, grooming, and routine household activities. Reinforce safe movement and activity techniques in the home setting. Patient Observation & Reporting Monitor patient response to therapy and report progress to the supervising OT. Communicate any functional changes or concerns. Caregiver Instruction Support Reinforce caregiver education provided by the supervising clinician. Help ensure consistency of therapy strategies between visits. Documentation Complete visit notes using a digital EMR system. Maintain accurate and timely records in compliance with regulations.
