Lead Case Manager IHS - Bridges at Kraemer (JR 5583) Title: Lead Case Manager – Bridges at Kraemer Location: Anaheim, CA 92806 Salary: $24.00 to $30.00 per hour Pay Frequency: 24 Pay Periods Per Year Position Type: Full Time, Non‑Exempt, Driving Required Shifts: 2 positions – Sun‑Thurs 7:00 am‑3:30 pm and Tues‑Sat 3:00 pm‑11:30 pm. About PATH: Since its foundation in 1984, People Assisting The Homeless (PATH) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation, case management, interim housing, and permanent supportive housing. About the job: The Lead Case Manager will assist clients in breaking the cycle of homelessness, supporting them from street to housing. The lead case manager is a seasoned social services professional who collaborates with a multidisciplinary team and the program manager. Responsibilities include: Ensure program compliance with contracts and partnership agreements in collaboration with the Quality Assurance Department. Act as a contact representative for program matters under the supervision of the program manager. Provide administrative oversight of the team and cover for the program manager when needed. Complete critical incident reports in accordance with PATH policies and contracts. Conduct client intakes, individualized needs assessments, and develop Individualized Service Plans (ISPs) addressing housing, income, and personal goals. Collaboration and teamwork skills, building positive relationships with colleagues, participants, and stakeholders. Excellent communication, both verbal and written. Creativity and initiative, taking proactive action and generating innovative solutions. Critical thinking and problem‑solving abilities, making logical decisions based on sound reasoning. Ethical conduct and professional boundaries. Preferred Qualifications: Three (3) years of experience in healthcare, case management, homeless services, or working with vulnerable populations. Minimum Qualifications: One (1) year of experience in healthcare, case management, homeless services, or working with vulnerable populations. Mandatory Requirements: All candidates must have employment eligibility verification, CPR/First Aid training, and successfully complete a tuberculosis test, background screening, and drug test as a condition of hire. Driving: Driving is an essential function. Candidates must have reliable transportation, a valid driver’s license, proof of insurance and vehicle ownership, and be able to qualify for PATH’s insurance coverage. Employment Eligibility: Candidates must be authorized to work in the U.S.; PATH does not sponsor or transfer visas. California‑Based Work Expectations: All work must be performed within California. PATH does not offer fully remote positions; work may include multiple designated sites, but no home‑based work.
