Job Summary This position may qualify for a sign‑on bonus. Full Time, Days Responsible for performing day‑to‑day implementation and follow‑up case management for patients in cardiac rehab. Provides individualized rehab program to meet personal and family needs of patients. The position is responsible for outpatient classes and requires an understanding of cardiovascular disease. Responsibilities Assesses patients’ physical, psychosocial, and educational needs on admission. Sets realistic goals. Individualizes patients’ plan of care. Performs no invasive cardiology tests. Documents patient care assessments. Qualifications Education: Required – Bachelor’s degree in EP or related exercise science field. Certifications: Required – AHA BLS.
