Hospice Registered Nurse Case Manager (RN)

divh2Now Hiring RN Case Manager/h2pStarting Salary: 90,000/Year/ppThe RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:/pulliCompleting an initial assessment of patient and family to determine hospice care needs/liliRegularly re-evaluates the patients nursing needs/liliUse assessment data to determine care plan needs/liliCollaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated/liliCounsels the patient and family in meeting end of life needs/liliInitiates appropriate preventative and palliative nursing procedures/liliCommunicates with the physician regarding patients needs and reports any changes in the patients condition/liliSupervises the Hospice Aides/li/ulpEducation: Associates Degree in Nursing/ppProfessional Experience:/pulliMust have a State Certified RN license/lili2 years experience as a professional nurse, hospice experience preferred/liliMust have a valid Drivers license and reliable transportation/liliMust be willing to travel/liliStrong verbal and written communication skills./li/ulpAscend Hospice - is part of the Ascend family of Healthcare Companies.
