Job Responsibilities

• Providing hands-on nursing care to residents in post-acute care settings
• Adhering to individualized resident care plans
• Reviewing and maintaining accurate resident charts and medical records
• Monitoring residents' physical and mental health and keeping a close watch on vital signs
• Verifying, administering, and tracking the effects of resident medications
• Assisting with IV setup, phlebotomy procedures, and immunizations
• Performing resident intakes and thoroughly documenting medical histories and health concerns
• Contributing to a safe, clean, and comfortable facility environment

Qualifications and Skills

• Applicable state-issued LPN license
• Successful completion of approved LPN program
• Minimum of 6 months of experience (LPN or comparable role)
• Ability to lift a minimum of 50 lbs
