Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
• Implement care plans and collaborate with interdisciplinary teams to ensure comprehensive care
• Conduct routine assessments to identify changes in resident condition and report findings to the nursing team
• Develop and maintain strong relationships with residents, families, and other healthcare professionals
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Maintain accurate and up-to-date records of patient care and treatment
