Lead Care Manager - Family-Centered ECM

Exceptional Parents Unlimited, Inc. is seeking a Lead Care Manager to provide family-centered care coordination for children and youth enrolled in Enhanced Care Management (ECM). The LCM will oversee service delivery, engage eligible members, and coordinate with providers to ensure seamless, non-duplicative care.
