Description: 60/40 Hospital/Outpatient split. Outpatient includes hands/upper extremity. Hospital includes general rehab. Adults of all ages.

Required: MN OT license Epic experience BLS

Temp: Will issue within 5-10 business days of receiving NBCOT and license verification (can issue sooner). Fingerprint results are NOT required for the temp to issue.

Perm: Allow 4-6 weeks for processing. Background check results alone can take 3-5 weeks to receive.

Traveler's primary residence must be at least 25 miles away from the facility

CNA License: MN license in-hand required

RN/LPN License: MN license in-hand or proof of application required

Vaccines: Medical/Religious exemption form required with submittal

Labs: Travelers are required to go to the lab within 48 hours of signing, or risk cancellation

Livescan Fingerprinting: MN residents and non-MN residents must commit to completing this within 48 hours of signing, or risk cancellation. All travelers MUST complete fingerprinting in person in MN on the Monday prior to their Monday start. This is a MN State Requirement NO EXCEPTIONS. Traveler is financially responsible for travel arrangements.
