Job Summary
The Physical Therapist Home Health (PT HH) position is located in a healthcare setting specializing in home health services. This role involves providing outpatient physical therapy care to geriatric patients with a focus on post-operative recovery, spinal conditions, and neurological disorders. The position is scheduled for daytime hours and requires active licensing and experience in Homecare Homebase (HCHB) systems.

Job Details

• Schedule: 8-hour shifts, Monday through Friday, with a minimum of 40 hours per week, starting 10/05/2026 and ending 01/04/2027.
• Required Qualifications: One year of recent relevant experience, current licensure, proficiency in HCHB (Homecare Homebase) for staff completing Start of Care (SOC), no first-time travelers, active driver's license and personal transportation, and successful completion of Monday orientation.
• Preferred Qualifications: Oasis experience required for staff completing SOC, home health experience, and familiarity with a variety of patient diagnoses common in geriatric populations.
• Additional Information: The caseload includes 5-7 patients per day, and all charting must be completed by 9 a.m. the following morning. The position involves travel within a territory covering parts of Walla Walla county, Columbia county, and Umatilla county. Flexibility for on-call rotation and RTO requests must be discussed during the interview.
