Travel Percentage: Up to 50% Job Shift: 2nd Shift (Based on Program) Job Category: Nonprofit - Social Services Adult Mobile Crisis Team Case Managers work closely with Madison County First Responders, First Responder Teams, Crisis Teams and Therapeutic Teams in partnership to provide every appropriate resource and support needed to eligible patrons as they transition from crisis to care and from care to recovery. This position requires organization and communication skills, the ability to connect and evaluate situations quickly, and knowledge of local providers. As the Case Manager, you are the link and liaison for your client. This is a part‑time, hourly position working Saturdays & Sundays with 7pm‑7am. Responsibilities Provide outpatient case management services to the seriously mentally ill population. Provide direct services to severely mentally ill clients to address unmet needs and provide information about continuum of care. Provide training on money management for meeting the client’s needs to obtain and maintain psychiatric stability in a residential setting. Provide follow‑up progress reports as needed to treatment team members. Evaluate client’s residential stability and assist those in need of a more stable environment. Provide basic living skills training when necessary and prepare client for less intensive level of care within the community. Assist clients with obtaining medication. Monitor medication compliance, maintain awareness of common side effects, and ensure client takes medication as directed on a regular basis. Provide traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc. Maintain high‑quality client documentation and records management, utilizing electronic charting. Monitor the care and services clients receive in a residential setting. Develop a positive working relationship with residential care manager/owner to meet client needs. Develop and implement an Individual Service Plan consisting of goals, resources, and interventions. Review client’s progress every three months and revise the Individual Service Plan as needed. Review SUN‑R six months after initial assessment and then at intervals of 12 months thereafter. Revise client’s goals once met or as progress occurs. Assist mentally ill clients through crisis situations and/or arrange for assistance by other professionals or personal caregivers. Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness. Complete other duties as assigned. Qualifications Bachelor’s degree. Acceptable 3‑year driving record & reliable transportation. Excellent verbal and written communication skills. Successful completion of State of Alabama Adult Case Management Training.
