Conduct comprehensive assessments of patients' medical, social, and psychological needs Collaborate with the treatment team to develop individualized care plans. Coordinate and monitor the delivery of services, including therapy, medical care, and support services Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey Provide education and support to patients and their families Qualifications Bachelor's or Master's degree in a related field preferred, e.g., Social Work, Psychology Clinical case management experience, preferably in a rehabilitation or healthcare setting Strong interpersonal and communication skills Knowledge of brain injuries and rehabilitation preferred About our Line of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
