Dialysis Social Worker - Care Planning & Patient Support

Fresenius Medical Care

Fresenius Medical Care seeks an entry-level Medical Social Worker to provide psychosocial services for center and home dialysis patients. You will assess psychosocial status, collaborate with the care team, and support education and referrals to community resources to improve quality of life. As part of an interdisciplinary team, you will document interactions and participate in quality and patient education activities, including advance directive discussions and transitions in care. #J-18808-Ljbffr Fresenius Medical Care
