Home Health Physical Therapist - One-on-One Care (NJ)

We are a PT owned and operated home care agency offering Medicare Part B in home physical therapy services! We provide one on one treatment to patients in the comfort of their own home. Role Description This is a Physical Therapist role for the towns within Essex County, surrounding Verona, NJ. The Physical Therapist will be responsible for evaluating and treating patients, developing individualized treatment plans, and documenting patient progress in their homes. Day-to-day tasks include providing hands‑on therapy, guiding rehabilitation exercises, and collaborating with other healthcare professionals to ensure comprehensive care. The Physical Therapist will also provide patient education and support to promote recovery and wellness in the comfort of the client’s home. Qualifications Proficiency in Physical Therapy and Rehabilitation Strong skills in Patient Care and Home Care Excellent communication and interpersonal skills Ability to work well within a team Bachelor's or Master's degree in Physical Therapy Valid NJ state license to practice as a Physical Therapist Experience in home health care is a plus #J-18808-Ljbffr Vitality Health Care
