Home Care RNEmployment Type: Full timeShift: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS!Location: Enfield, Windsor RegionsStatus: Full TimeBonus: $10,000 sign-on bonus paid out over two years.Position Purpose:Our Home Care RN's provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization's policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. They also provide functional support/supervision to Home Healthcare Aides and LPNs as needed and therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.Position Details:Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year.
