Requirements: BLS; prior travel experience; 2 years of CM experience; acute care experience; FL/Compact nursing license; recent Nursys profile; 5 years of experience or degree accreditation on profile
Responsibilities:

• Provide case management services within the emergency department for readmission initiatives.
• Support acute care patient needs through care coordination and resource management.
• Collaborate with medical teams to facilitate patient discharge planning and follow-up.
• Function as a core staff member, including floating to sister campuses as needed.
• Adhere to all documentation deadlines and profile requirements, including demographic information.
