Referral Management Coordinator

Consult Tracking Coordinator
Educate staff, healthcare professionals and beneficiaries on the consult tracking procedures
Maintain suspense database for contacting civilian healthcare professionals to acquire consult results
Work closely with network providers to ensure contact information and required chart notes are sent and received in a timely manner to provide quality clinic visits
Independently, identify and notify staff of items requiring urgent attention and processing to ensure best healthcare outcomes
Maintain positive and clear communication with nurse case managers, group practice managers, providers, and the clinical support division to ensure the patients needs and hospital requirements are followed throughout the process
