Hours for this Position Monday-Friday Full time TBD Advantages of this Opportunity Competitive salary, 53K-63K negotiable based on relevant experience Benefits offered, Medical, Dental, and Vision Fun and positive work environment Duties Provides case management services to members with chronic or complex conditions Conducts assessment of the member, member’s family/caregiver, hospital staff, physicians and other providers Develops and implements a case management plan Performs ongoing monitoring of the plan of care to evaluate effectiveness Evaluates effectiveness of the care plan and modifies as appropriate to reach optimal outcomes Qualifications MUST be an RN with 2+ years of experience 1-2+ years of Case Management experience or Hospital background coordinating care, discharge planning etc. Interqual or Milliman experience preferred Experience working with Complex/Chronic conditions Must have clear driver’s license Additional Information Are you a Registered Nurse in Naples area looking for a new opportunity with a prestigious healthcare company?
