We are currently looking for someone available to work PRN and assist with client assessments as needed.We assist seniors in and around the Dallas area, and specialize in Alzheimer's / Dementia care, hospice, and general geriatric care. These tenets form the cornerstone of our approach to caregiving, reflecting our unwavering dedication to providing the highest quality of life for our senior clients.Responsibilities and DutiesConduct in-home and/or facility-based client assessments as needed to determine care needs and eligibilityCollaborate with intake, case management, and nursing teams to ensure seamless care coordinationEvaluate client conditions and recommend appropriate levels of care and servicesMaintain accurate and timely documentation of all assessments and clinical findingsCommunicate effectively with clients, families, and referral sources to support care planningEnsure compliance with all state regulations and company policies during assessmentsSupport care transitions from hospitals, rehab facilities, or other care settingsBuild and maintain professional relationships with referral partners and healthcare providersProvide clinical insight and support nursing and caregiving staff as neededParticipate in case reviews and contribute to ongoing quality improvement initiativesTravel within the Dallas area to complete assessments as requiredSchedulePRN (as-needed) position with flexible schedulingHours will vary based on client assessment needs and referral volumeOpportunity to pick up shifts during weekdays, evenings, and occasional weekendsIdeal for individuals seeking supplemental income or flexible work alongside another roleAdvance notice will be provided when possible; some same-day or next-day availability may be requestedRequirementsMust have a full, active license as a Registered Nurse.Minimum of two years related work experience in a geriatric healthcare setting required.Flexibility as needed when it comes to assessments and scheduling trainingsDemonstrates effective verbal and written communication skillsAbility to work independentlyDemonstrates initiative, problem solving ability, adaptability and flexibilityClinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination.
