Shift: Days
Hours Per Week: 40 hours Hiring Range : $110,000-$130,000 with certification. The actual compensation for this position will be determined based on experience and other factors permitted by law. Responsibilities: Administrative: Establishes goals and objectives for his/her area in congruence with the assigned department's goals and objectives on an annual basis. Ensures that development of policies, procedures, and standards of care are maintained. Provides coaching and guidance to nursing and other clinical staff and acts as a role model and resource person. Always ensures and maintains patient/family confidentiality. Ensures nursing care is within scope of practice and that documentation is consistent with best practice standards. Reviews and audits charts to verify order authentication as appropriate. Ensures compliance with all regulatory requirements.
Clinical Care : Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. Determines eligibility of patients for entry into clinical trials based on findings. Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care. Interprets and evaluates findings of studies/tests.
Communication and Documentation : Relays appropriate information regarding patient care to the collaborating physician. Dictates follow-up letters to referring physicians with summaries of treatment, response, and plan. Dictates or writes patient history, admissions, care plans, progress, and discharge notes. Multidisciplinary Relationships : Interacts with leadership team as well as support staff to ensure provision of optimal patient care. Conducts or participates in daily multidisciplinary rounds and ensures appropriate and quality care for assigned patient. Performs patient triage and determines the urgency of physician's evaluation. Works effectively with others. Collaborates with other health disciplines to ensure continuity and quality of care. Shares clinical expertise with professional and supportive personnel. Collaborates with referring agencies.
Patient and Family Education : Educates patients and families about the disease process and treatment. Provides information to patients and families related to symptom management and psychosocial response to the diagnosis of cancer and related treatment. Provides symptom management for side effects related to treatment. Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care. Workflow: Maintains timely schedule for patient appointments. Responds promptly to urgent patient needs and issues. Manages time and works effectively with other team members. Evaluates patients in a timely manner. Utilizes EMR technology effectively and appropriately. Effectively prioritizes patient care issues. Professionalism: Embraces and reflects the mission and goals of St. Mary's Healthcare. Advocates for patients, caregivers and families. Displays a positive attitude towards job, role and responsibilities. Treats all team members/clinical staff with respect. Displays positive interpersonal skills with staff, patients and families.
Leadership/Mentoring: Applies leadership skills as clinical role model, leading clinical skill development, fostering critical thinking skills, promoting safe practice and acting as a liaison/consult as needed Provides guidance and leadership to nursing staff. Coordinates the education and development of new nursing staff.
Committees/Participation: Actively participates and promotes professional practice by participating in applicable committees. Continues education through workshops, conferences, in-services and self-study. Education, Training & Experience:
- MSN degree required.
- Licensed as Nurse Practitioner in the State of New York prior to start date and/or transfer date.
- Specialized training and certification through Hospice and Palliative Nurses Association (HPNA) or the American Nurses Credentialing Center (ANCC) required.
- Ability to organize and establish day-to-day priorities while utilizing critical thinking skills in all aspects of the job.
- Must be able to multitask while remaining professional, focused, composed and positive.
- Excellent customer service skills and must display integrity, friendliness, and compassion.
- Must be able to establish an appropriate and effective rapport with others.
- Must be flexible to take initiative and embrace new opportunities to grow both personally and organizationally.
