Job Description

Job Summary:

The Physical Therapist provides in-home outpatient services within a home health setting, focusing on patient mobility, functional independence, and rehabilitation goals. The role involves visiting multiple patients daily, documenting progress, and collaborating with healthcare teams to optimize patient outcomes.

Responsibilities:

• Assess and develop individualized treatment plans for adult patients in home health environments.
• Deliver therapeutic interventions through home visits, ensuring adherence to care plans.
• Document patient progress, treatments, and assessments accurately within required timeframes.
• Travel up to 75 miles daily between patient locations, maintaining reliable transportation and insurance.
• Participate in periodic office meetings and collaborate with interdisciplinary teams to review patient care.
• Manage a caseload of approximately 5-6 patients daily, aiming for productivity points of 30 per week.

Qualifications:

• Hold a valid state license as a Physical Therapist with current certification.
• Have at least 1 full year of recent adult home health experience.
• Proficient in OASIS documentation and recent use (within 6 months) is required.
• Demonstrate ability to travel extensively within the assigned territory.
• Possess current CPR/BLS certification.
• Be willing to work varied schedules, including potential weekend and on-call rotations.
