Genesis Medical Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing medical team providing primary care services to geriatric and post-acute patients within a skilled nursing facility in Oneonta, NY.

This is an excellent opportunity for a provider seeking a consistent 34-day onsite schedule, strong earning potential, and the ability to practice independently while being supported by an established medical team.

Position Highlights

• 34 days per week onsite at the facility
• Wednesday coverage is already in place
• Consistent daytime clinical schedule
• Telephone call coverage required on scheduled workdays from 7:00 AM9:00 PM, with the possibility of coverage extending until 11:00 PM

Responsibilities

• Provide primary, post-acute, and long-term care to skilled nursing facility residents
• Perform admissions, follow-up visits, assessments, and chronic disease management
• Evaluate changes in condition and address acute medical concerns
• Review labs, imaging, medications, and clinical reports
• Collaborate with physicians, nursing staff, specialists, families, and facility leadership
• Answer clinical calls on scheduled workdays between 7:00 AM and 9:00 PM, with possible availability until 11:00 PM
• Provide guidance for urgent clinical needs during assigned coverage hours
• Help reduce unnecessary hospitalizations through proactive patient management
• Complete documentation and EMR charting accurately and promptly

Requirements

• Active New York NP or PA license
• Availability to work 34 days per week onsite
• Willingness to provide telephone call coverage on scheduled workdays
• Strong clinical judgment and the ability to work independently
• SNF, nursing home, primary care, internal medicine, or geriatric experience preferred
• Comfortable managing medically complex patients
• Reliable transportation
