Job Summary:
Physical Therapist Outpatient - Martinsville, Indiana. This outpatient position involves evaluating patients, developing and implementing treatment plans, and ensuring effective recovery through functional therapy programs. Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; located in Indiana or willing to commute; valid state licensure in good standing; and a minimum of 1 year of relevant experience.
• Demonstrates behaviors and attitudes aligned with the organization's Mission Statement and Core Values.
• Completes comprehensive evaluations and delineates goals focused on patient discharge within an established length of stay.
• Documents clinical information accurately and efficiently, including treatment plans, goal writing, and compliance with applicable guidelines.

• Maintains professional licensure, adheres to regulatory standards, and pursues ongoing professional development.
• Understands and applies organizational performance improvement methodologies.
• Promotes safety awareness and practices in all aspects of patient and workplace safety.
