You’re Not Just a Nurse – You’re a Compassionate Caregiver

As our Licensed Practical Nurse , you will deliver meaningful, hands-on care that brings real comfort to patients and families during their most vulnerable moments. This role goes beyond clinical tasks – you’ll be the steady, compassionate presence that makes all the difference.

Licensed Practical Nurse Responsibilities

• Deliver direct patient care : Provide skilled nursing care including wound care, medication administration and pain/symptom management under the supervision of an RN Case Manager.

• Be the Patient’s Advocate : Support each patient’s unique needs, preferences, and wishes while providing compassionate guidance to their loved ones.

• Build Meaningful Connections : Develop trusting relationships with patients and families, offering emotional support and reassurance when it’s needed most.

• Contribute to Care Planning : Assist in implementing and updating individualized care plans that prioritize comfort and quality of life.

• Document with Care : Accurately record patient progress, observations, and care activities to support continuity of care.

 

Your Impact Zone

As an LPN with Cherish Hospice, you’ll travel throughout Northern Indiana, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals – wherever they feel most comfortable.

Required Qualifications

• Active LPN License in the state of Ohio

• Valid Driver’s License

• 1+ year of experience in hospice, palliative care, or a related clinical setting

 

Preferred Qualifications

• BLS Certification

• Hospice and Palliative Medicine background

• Experience working under RN supervision in a home health or hospice environment
