Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home health nursing care in accordance with physician orders, agency policies, and procedures. Coordinates patient care activities, supervises home health aides and licensed practical nurses, and ensures timely documentation and patient outcomes.

Responsibilities:

• Coordinate and deliver personalized nursing care to patients in their homes, adhering to physician orders and clinical protocols.
• Supervise and evaluate the performance of home health aides and licensed practical nurses, ensuring quality care delivery.
• Document patient assessments, treatment plans, and progress accurately within the required timeframe using HomeCare HomeBase (HCHB) and Oasis for applicable clinicians.
• Travel up to 75 miles daily between patient homes, managing schedule flexibility to accommodate patient needs and workload.
• Participate in periodic office meetings and maintain communication with the Clinical Manager regarding patient status and care plans.
• Comply with all agency policies, state regulations, and safety standards, including current CPR and BLS certifications.

Qualifications:

• Valid RN license in the state of Maryland, in good standing, with license in hand.
• Minimum of 1 year of recent adult home health or hospice care experience.
• Proficient in HomeCare HomeBase (HCHB); Oasis proficiency required for RNs.
• Reliable transportation, valid driver's license, and current auto liability insurance.
• Ability to travel up to 75 miles from the care center and accept flexible patient scheduling.
• Must be able to complete documentation within 24 hours and accept full patient load regardless of travel distances.
