Job Summary:
Provide specialized social work services within a hospital setting to support patient wellbeing and discharge planning. Focus on delivering compassionate, evidence-based interventions aligned with clinical standards.

Responsibilities:

• Assess patients' psychosocial needs and develop individualized care plans.
• Collaborate with healthcare teams to ensure comprehensive patient support.
• Document patient interactions and progress accurately in medical records.
• Assist patients and families with emotional support, resource connections, and discharge processes.
• Advocate for patients' mental health and safety throughout their hospital stay.

Qualifications:

• Hold a valid Georgia Licensed Certified Social Worker (LCSW) credential.
• Possess a minimum of 2 years of recent experience in a hospital or acute care setting.
• Demonstrate proficiency in clinical psychosocial assessments and interventions.
• Have effective communication skills for multidisciplinary team collaboration.
• Exhibit strong organizational skills to manage complex patient cases.
