Provide specialized social work services within a hospital setting to support patient wellbeing and discharge planning. Focus on delivering compassionate, evidence-based interventions aligned with clinical standards. Responsibilities:
- Assess patients' psychosocial needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure comprehensive patient support.
- Document patient interactions and progress accurately in medical records.
- Assist patients and families with emotional support, resource connections, and discharge processes.
- Advocate for patients' mental health and safety throughout their hospital stay.
- Hold a valid Georgia Licensed Certified Social Worker (LCSW) credential.
- Possess a minimum of 2 years of recent experience in a hospital or acute care setting.
- Demonstrate proficiency in clinical psychosocial assessments and interventions.
- Have effective communication skills for multidisciplinary team collaboration.
- Exhibit strong organizational skills to manage complex patient cases.
