Physical Therapist (PT) - Must Be Licensed

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team! This is a part time opportunity. This is a direct hire/perm opportunity. Responsibilities Conduct assessment of referred patients requiring rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, restrictions and potential for rehabilitation. Establish and administer a treatment program with specific goals based on patient capacity and tolerance under direction of the Physician. Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families in appropriate physical therapy methods. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduated from an accredited school. Current state license as a Registered Physical Therapist / PT or actively pursuing licensure. Current CPR certification.
