Job Summary As a Medical Provider for our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. This role demands autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. All activities will be guided by quality measures provided by Atlas, emphasizing our commitment to high standards of care. Desired Skills & Qualifications Must have at least one (1) year of primary care experience Valid Licensure as a Nurse Practitioner or Physician Assistant in State of Practice Ability & Desire to work independently, and drive between communities. We currently use Elation as our EMR Effective verbal & written communication & time management skills Valid AZ Fingerprint Clearance Card Current CPR Certification & Proof of Negative TB Test Make a Difference with Atlas!
