Physical Therapist - Full Time
Apex Home Health, Hospice & Home Care
Join our team as a Hospice Physical Therapist to provide compassionate, comfort-focused therapy services aimed at enhancing patients' quality of life at the end of life. Key Responsibilities Deliver physical therapy per physician’s plan and state regulations, focusing on comfort, mobility, and safety. Assess patients’ functional status, mobility, and environment; develop and implement therapy plans. Educate patients, families, and caregivers on therapy techniques and use of devices. Supervise Physical Therapy Assistants and provide consultation and education to staff. Document clinical notes, progress, and discharge summaries; participate in care planning and interdisciplinary meetings. Identify additional home health needs and make appropriate referrals. Qualifications Bachelor’s or master’s degree in physical therapy from an accredited program. Licensed to practice as a physical therapist in the respective state. Preferred two years of relevant experience, especially in home health or community settings. Strong communication and organizational skills.
