Responsibilities Job Summary The Physical Therapist is responsible for the evaluation, administration, and oversight of evidence‑based physical therapy services provided to individuals with disabilities and/or functional limitations. They use specialized skills to promote goals of prevention of dysfunction, restoration of function, and/or reversal of the progression of pathology to enhance movement quality and daily living skills. Essential Functions Implements an individualized treatment plan using evidence‑based therapy techniques, including therapeutic exercises, manual techniques, functional mobility training, ADL training, and modalities. Provides verbal and written instructions to patients, caregivers, and family members as needed. Evaluates patients per the Department Operating Policy, synthesizes data from evaluations, establishes comprehensive treatment plans with measurable goals, and a defined discharge plan. Performs ongoing evaluation to assure effective service provision and documents accordingly. Maintains accurate documentation in the patient’s medical record: evaluations, treatment notes, treatment plans, progress reports, discharge summaries, and daily charges. Completes billing per department procedures within established timeframes. Collaborates with the interdisciplinary team to clarify plans, review patient status, and coordinate with medical personnel. Provides proactive feedback to case managers and key personnel concerning discharge needs and equipment requirements. Participates in interdisciplinary rounds and communicates to facilitate optimal outcomes. Delegates appropriate cases to physical therapist assistants, supervises their care, and provides updated treatment plans and instructions. Serves as a resource for therapy‑related issues across departments. Contributes to department improvement efforts, recruitment, and retention of staff through positive role‑modeling. Participates in staff meetings, training, utilization review programs, committees, and preceptor/mentor programs. Education Master’s Degree or Doctorate in Physical Therapy from an accredited university approved by the APTA – Preferred Bachelor’s Degree in Physical Therapy from an accredited university approved by the APTA – Required Experience Network float position: six (6) months recent experience – Preferred 6 months experience – Preferred Licenses and Certifications American Physical Therapy Association (APTA) – Preferred Basic Life Support (BLS) – Required AZ State Board of Physical Therapy and/or PT Compact License – Current license issued by the Arizona Board of Physical Therapy in good standing, or Compact state licensure upon hire – Required #J-18808-Ljbffr HonorHealth
