Position Summary:

• We are seeking a compassionate and patient-focused Medical Primary Care Provider (MD/DO, PA, or NP) to join a well-established outpatient practice. The provider will deliver comprehensive primary care services, focusing on preventive medicine, chronic disease management, and continuity of care while collaborating with an integrated medical and behavioral health team.
• This position does not include initiating ketamine treatment or serving as the primary prescriber for ketamine-assisted therapy. The provider may assist in monitoring the overall medical stability of patients receiving behavioral health services in collaboration with other designated providers.

Key Responsibilities:

Primary Care:

• Conduct comprehensive patient evaluations, including medical histories and physical examinations.
• Diagnose and treat common acute and chronic medical conditions.
• Develop evidence-based treatment plans.
• Provide preventive healthcare, routine screenings, immunizations, and patient education.
• Manage chronic diseases such as hypertension, diabetes, asthma/COPD, thyroid disorders, and hyperlipidemia.
• Order and interpret laboratory tests, imaging, and diagnostic studies.
• Prescribe medications within scope of practice.
• Maintain timely and accurate EMR documentation.

Care Coordination:

• Coordinate referrals to specialists and diagnostic services.
• Collaborate with behavioral health providers and multidisciplinary care teams.
• Review laboratory and imaging results and ensure appropriate follow-up.

Behavioral Health Collaboration:

• Provide primary care oversight for patients also receiving behavioral health services.
• Coordinate with psychiatric providers regarding relevant medical history and treatment considerations.
• Support integrated patient care while following established clinical protocols.

Professional Responsibilities:

• Participate in interdisciplinary meetings and case discussions.
• Follow all clinical, regulatory, and compliance requirements.
• Maintain professional licensure, DEA registration (if applicable), and continuing education.

Qualifications:

Education & Licensure:

• MD or DO with an active state medical license, or
• Certified Physician Assistant (PA-C) with an active state license, or
• Nurse Practitioner (FNP, ANP, or other primary care certification) with an active APRN license.
• Active DEA license or ability to obtain one before start date (if prescribing controlled substances).

Experience:

• Three or more years of outpatient primary care, family medicine, or internal medicine experience preferred.
• Strong new graduates with relevant clinical training are encouraged to apply.
• Experience in integrated medical and behavioral health settings is a plus.

Preferred Skills:

• Strong diagnostic and clinical decision-making abilities.
• Experience managing chronic diseases in an outpatient setting.
• Excellent communication and patient education skills.
• Ability to collaborate effectively within a multidisciplinary healthcare team.
• Experience using electronic medical record (EMR/EHR) systems.

Physical Requirements:

• Ability to stand and walk throughout the workday.
• Ability to lift up to 25 lbs. with or without reasonable accommodation.
• Ability to perform routine clinical procedures such as injections, IV placement, and phlebotomy.
• Comfortable working in an outpatient clinical environment with direct patient care.
