Part-Time Physical Therapist — W-2 Employee

Front Street Medical is seeking a dependable, patient-focused New York-licensed Physical Therapist to join our established multidisciplinary medical practice.

We have offices in Lindenhurst and Hempstead, New York , offering physical therapy, medical and chiropractic services in a supportive team environment.

Current Schedule:

Our immediate need includes:

• Tuesday, 3:00–7:00 PM — Lindenhurst.
• Thursday, 3:00–7:00 PM — Lindenhurst.
• Friday morning — Hempstead.
• Optional Saturday hours, 10:00 AM–2:00 PM — Lindenhurst.
• Additional weekday and coverage hours may become available.

We are open to one therapist covering the complete schedule or qualified therapists seeking a portion of these hours. The position can begin part-time and has the potential to grow. We are also flexible to some degree with evening hours and can discuss slight adjustments to accommodate the right candidate’s availability.

Employment and Compensation:

• W-2 employee position.
• $50–$72 per hour, based on experience, availability and qualifications.
• Consistent scheduled hours.
• Established patient base.
• Administrative and front-desk support.
• Multidisciplinary office environment.

Qualifications:

• Active and unrestricted New York State Physical Therapist license.
• Current individual NPI.
• Current CAQH profile.
• Strong clinical documentation and communication skills.
• Reliable and able to maintain an agreed schedule.
• Outpatient orthopedic experience preferred.
• No-Fault and Workers’ Compensation experience is helpful but not required.

Current participation with Medicare and major New York commercial insurance plans is strongly preferred. Please include a list of the insurance plans with which you currently participate. Our credentialing team will assist with adding the selected therapist to Front Street Medical’s group and service locations.

This is a W-2 employee position and is not an independent-contractor assignment.
