Memorial Specialty Nurse Practitioner/Physician AssistantThe Memorial Specialty Nurse Practitioner/Physician Assistant renders medical care to patients within the specialty care setting of clinic offices and is responsible for management of acute episodic problems and stable chronic conditions in addition to health promotion and disease prevention in conjunction with physicians and other healthcare staff. The nurse practitioner communicates care needs with patients, families, and the healthcare team for the continuance of care. The nurse practitioner identifies and implements best practices to achieve improved outcomes and greater effectiveness of patient care.ResponsibilitiesProvides specialty patient care services including management of acute episodic problems and stable chronic conditions, health promotion and disease prevention as allowed by scope of practicePerforms history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical problemOrders appropriate diagnostic studies and correctly interprets the resultsPerforms preventative health assessments, screening, immunizations and patient careProvides direct treatment and management of health conditions via referral to other healthcare providers or community resourcesReviews daily plan of care and analyzes complex patient care concerns to modify the plan of care based on the patients response to treatment and evaluates the overall plan for effectiveness; Collaborate with supervising physicians concerning patient care as appropriateInstitutes care that might be required to stabilize a patients condition in an emergency or potentially life-threatening situationManages general medical and surgical conditions based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other relevant treatment modalitiesEvaluates patient medication regimen and prescribes pharmacological agents as allowed by Federal and State laws and authorized by licensing entityDemonstrates proficiency in procedural skills pertinent to practice areaMaintains accurate, complete, concise and timely documentation in patients medical recordCorrectly perform advanced procedures as allowed by state nurse practice actCommunicates patient/family health care needs as appropriate for continuance of careCommunicates and interacts with patients, families, and visitors of all age groupsIdentifies expected patient outcomes in collaboration with patient, family, and other professionalsCounsels patients and families regarding medical processes, management of the health illness/condition, medication, nutrition, and health promotionCounsels patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over the counter medications and herbal remediesInitiates timely consultation and referral when the problem exceeds scope of practice and/or expertiseServes as a resource for patients, families, and staffProvides ongoing identification of best practices and implementation of those changes that lead to improved outcomes and greater effectivenessFunctions as a consultant and change agent for practice issues and quality improvement activitiesProvides ongoing assessment and facilitation of Core Measure compliance, Joint Commission, and performance improvement efforts and implementation of appropriate changeContribute to the professional development of peers, colleagues, and others upon requestCritically evaluate and modify existing practice based upon current research findingsQualificationsNurse Practitioner:Education: Master's Degree from Nursing program from an accredited school of nursingExperience: Two (2) years Clinical experience as a staff Registered Nurse (RN).Licensure: Certification as a Professional Nurse Practitioner in Mississippi.Physician Assistant:Education: Graduate from an accredited Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) with a Master's Degree.License: Certified by the National Commission on Certification of Physician Assistants (NCCPA) with Current license to practice in the State of Mississippi as a Physician Assistant.Skills:Problem solving for patients with complicated disordersInstructing using adult learning methodsOral and written communicationGood judgment and quick decision-making abilityExcellent interpersonal skills while understanding the needs for a positive patient experienceJob InfoJob Identification 28828Job Category Advanced Practice ProvidersJob Schedule Full timeJob Shift DayLocations Coast Heart Institute 4215 15th Street
