Job Location: Siloam Springs, AR 72761 Position Type: Full Time Job Category: Direct Support Professionals Pay: $14.50 per hour Education Level: High school diploma or general education degree (GED) SUMMARY Responsible for implementing habilitation and supportive living plans for participants as developed by an interdisciplinary team and ensuring a safe and healthy environment within the community setting. Position guarantees a minimum of 32 hours per work week with flexibility to work whenever required. ESSENTIAL DUTIES AND RESPONSIBILITIES Coordinate services for clients. Work closely with the Service Manager to ensure quality health care for clients, including proper nutrition, exercise, mental and physical health areas. Coordinate with Service Manager regarding concerning client needs or concerns. Monitor all client medications, adhering to guidelines and documenting usage, counting medications, and other tasks as assigned. Follow all financial procedures for client checking accounts and personal/grocery shopping while supervising these activities. Provide transportation as needed for daily client activities. Complete necessary paperwork for billing Medicaid transportation as allowable. Conduct oneself in a professional manner at all times. Assist clients with personal hygiene, dressing, clothing maintenance, laundry, and other related tasks. Assist clients in cooking meals when required. Complete all documentation forms required by FCC, including progress notes, behavior reports, and marks reports. Assist in maintaining facility and grounds in a clean, orderly, and well‑maintained fashion. Assist in maintaining vans in good working order, including cleaning and preventive maintenance as scheduled. Maintain a log of all monthly emergency drills. Attend training sessions, in‑town or out‑of‑town, as required by FCC and licensing agencies. In case of emergency, take necessary steps and notify the Service Manager, Regional Manager, or appropriate person promptly. Keep a cell phone on hand when transporting consumers for emergency situations. Serve as driver for participant day trips to various state locations, driving as needed on a daily basis; undergo periodic driving tests and demonstrate competency with passenger vans. May be required to use personal vehicles with adequate liability coverage. Must possess a valid driver's license and clean MVR. This is a safety‑sensitive position. Maintain confidentiality of all information received regarding Friendship Community Care, Inc. and the clients, in compliance with the Friendship Community Care Non‑Disclosure Policy and HIPAA Policies. QUALITY ASSURANCE & COMPLIANCE Follow the Code of Conduct and Compliance Investigation Procedures established during new employee training. Maintain and complete consumer progress notes daily or as needed on the prescribed forms. Keep consumer records updated weekly, monthly, quarterly, and yearly according to funding source requirements. WORK ENVIRONMENT Work environment is moderately noisy. QUALIFICATIONS High school diploma or general education degree (GED), or 1–3 months of related experience or training, or an equivalent combination of education and experience. Valid driver's license. Language Skills: Ability to read and comprehend simple instructions, short correspondence, and memos; ability to write simple correspondence; ability to present information in one‑on‑one and small group settings. Mathematical/Computer Skills: Ability to add, subtract, multiply, divide, compute rates, ratios, percentages, and interpret bar graphs. Reasoning Ability: Apply commonsense understanding to follow written, oral, or diagram instructions and solve problems involving several concrete variables in standardized situations. PHYSICAL DEMANDS Regular use of hands to finger, handle, or feel objects, tools, or controls; talk or hear. Frequently required to stand, walk, walk with reach, sit, climb or balance, stoop, kneel, crouch, or crawl; ability to taste or smell. Occasional lifting of up to 50 pounds.
