Hospice Social Worker — Palliative Care & Family Support
Chapters Health System
Chapters Health System is seeking a Social Worker to provide psychosocial support to patients and families across Florida Keys & North Florida. The role requires MSW CSWE accreditation, strong communication, crisis intervention skills, and the ability to travel to off-site locations. The Social Worker will collaborate with hospice, palliative care, and home health teams to develop care plans, assist with advance directives, and support bereavement services as needed. #J-18808-Ljbffr Chapters Health System
