• Provide skilled nursing visits in the home; manage and update Plan of Care (POC)
• Educate patients/families, coordinate with interdisciplinary team
• Maintain compliant documentation and continuity of care

On-Call Registered Nurse (RN) - Home Health Requirements:

• Active RN license
• 2+ years home health experience (required)
• Homecare Homebase (HCHB) experience (required)
• OASIS experience (required)
• CPR + driver’s license + reliable transportation/insurance
