Duties & Responsibilities Program Facilitation & Community Outreach Coordinate and assist with core program operations, including patient enrollment, workshop facilitation, and daily activities. Manage schedules for classes and workshops while coordinating coverage across various program sites. Assist with outreach initiatives and referral programs, including conducting workshops and educational classes. Support language access by providing translation assistance for monolingual patients. Assist in coordinating partner meetings by gathering information, taking detailed notes, and managing follow-ups. Review and maintain accurate patient information, operational documentation, records, and program reports. Conduct regular program assessments, site visits, and community surveys. Handle donation collections and maintain accurate financial reporting. Order, receive, and maintain central inventory and program supplies. Provide administrative support and coverage as Staff on Duty as needed. Ensure strict adherence to all operational procedures, contract specifications, and regulatory standards. Travel to other local service sites as needed within city limits. Required Qualifications Education & Experience Bachelor’s degree or equivalent years of professional work experience. Language & Technical Capabilities Required bilingual fluency in English/Spanish or English/Chinese. Familiarity with Microsoft Office Suite and database management software. Excellent verbal and written communication skills with the ability to follow detailed instructions and work independently. Ability to stand for prolonged periods and occasionally lift up to 50 lbs. Desired Qualifications Minimum of one (1) year of experience working directly with older adults or senior citizens. Minimum of one (1) year of experience and comfort engaging in community outreach efforts. Certification as a Diabetes Educator or Health Educator is a plus. Location and Work Type Full-Time Position.
