Primary Care Physician

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for this population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, working to build trusting relationships and coach patients towards better health.

- Outcomes-focused, value-based organization
- PCPs lead a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients
- Engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources for homebound patients or those in hospital, SNF, or LTC
- PCPs will have an active role in the management of their center and will help cover for other providers
- PCPs are expected to take an active role in recruiting patients for the center and additional providers for the company
Responsibilities
- Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
- Engage with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Lead their care team for patients able to come to the office
- Adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures
- Participate in risk and quality management programs, clinical meetings, and other required meetings that promote patient health and company goals
- Perform other duties as assigned and modified at manager’s discretion
Compensation
- $221,141 - $315,915 Salary
- This position may also be eligible for a bonuses or commissions

Requirements

- Ability to act as a health coach, helping patients set and achieve health goals
- Ability to simplify and prioritize appropriately for patient behavior change and team performance goals
- Ability to function both independently and in collaboration with other health care professionals
- Availability and accessibility for patients to build trust, including being open for walk-in visits and timely response to phone calls and messages
- Service Orientation: provide care as they would for a family member or themselves
- Evidence Based Medicine: remain updated on evidence-based medicine, lifestyle, and nutrition interventions
- Physician Leadership: continuously work to develop and improve leadership skills
- Quality: willingness to work with care teams towards achieving high quality outcomes and continuously improve practice and center operations
- Influence: competently and compassionately influence patients, teams, and self to achieve best outcomes
- Self-Care: adequately care for themselves physically, mentally, socially, and spiritually
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
