What You'll Do

As an On Call LVN, you'll play a vital role in supporting patients during nighttime hours. You'll work closely with RNs and physicians to ensure continuity of care and comfort for our patients.

Key Responsibilities:

• Follow and implement the patient's plan of care as directed by the RN
• Provide timely and accurate documentation
• Conduct assessments and report findings to the care team
• Collaborate with interdisciplinary team members
• Use medical equipment and supplies efficiently
• Engage in ongoing professional development

What We're Looking For

• Valid California LVN license
• Minimum 2 years of direct patient care experience
• At least 1 year of hospice experience preferred
• Current CPR certification
• Reliable transportation with valid driver's license and insurance
