Certified Occupational Therapy Assistant (COTA) - Full-Time

Location: Newton, IL
Facility: Newton Care and Rehab Center
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team at Newton Care and Rehab Center in Newton, Illinois.

This full-time Occupational Therapy Assistant opportunity offers the chance to provide meaningful patient care in a supportive, collaborative skilled nursing facility where your clinical expertise makes a daily impact.

As a COTA , you will work alongside a licensed Occupational Therapist to help adult and geriatric patients improve independence, safety, and quality of life through individualized occupational therapy interventions.

• Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans designed to improve patients’ functional abilities and independence.
• Assist patients with activities of daily living (ADLs), mobility, coordination, strength, and safety awareness.
• Monitor patient progress and communicate updates with the supervising Occupational Therapist.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, safety strategies, and adaptive equipment.
• Provide compassionate, patient-centered occupational therapy services.

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Illinois Certified Occupational Therapy Assistant (COTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
