RN - Data Entry Nurse

Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS) nurse to support our Healthcare & Medical Services team within our senior care and post-acute settings. As an MDS RN, you will play a key role in coordinating and completing comprehensive resident assessments, ensuring accurate and timely MDS/RAI documentation that reflects residents’ clinical status, functional abilities, and care needs. You will work closely with an interdisciplinary team—including physicians, therapists, social workers, and nursing staff—to develop, update, and evaluate individualized care plans that promote quality outcomes, safety, and resident-centered care. In this role, you will review clinical documentation, interview residents and families, and collaborate with care teams to gather all necessary data for MDS submissions, while monitoring for accuracy, consistency, and regulatory compliance. You will help ensure adherence to federal and state long-term care regulations, payer requirements, and Sanford Health policies, supporting proper reimbursement and case-mix accuracy. The MDS RN will provide education and guidance to nursing and frontline staff on documentation standards, care planning, and quality measures, fostering a culture of continuous improvement, compassionate care, and innovation. You will monitor quality indicators, participate in audits and performance improvement initiatives, and communicate findings and recommendations to leadership. This position requires strong clinical assessment skills, attention to detail, and proficiency with electronic health records and MDS software.
