Case Manager

Case Manager ICTT (Full Time) Location: Woonsocket, RI Starting pay rate: $25/hr Overview Community Care Alliance, located only 15 miles from Providence, RI, seeks compassionate individuals who want to make a difference in someone else's life. ICTT (Intensive Community Treatment Team) Case Managers are an integral part of our Community Support and Nursing Department connecting individuals and families to various providers and resources so that individuals can live as independently and as fully integrated within the community as possible. Responsibilities Provide clinical and support services for individuals and families with long‑term needs associated with mental illness and substance use. Work with case managers and multi‑disciplinary teams in the home and community to help people maintain their health and well‑being. Develop, design, and implement treatment plan goals with clients to promote recovery. Qualifications Demonstrated strong writing, communication, computer, and documentation skills. Ability to multitask, be organized, and manage time effectively. Ability to transport clients in their vehicles for medical and community appointments. Ability to work independently or as part of a team to best assist clients. Requirements Associate’s degree in a human services or related field. Approved driver’s record and reliable transportation.
