Wellness Nurse (LPN) SIGN-ON-BONUS

Job Description Reporting to the Director of Health and Wellness, the Wellness Nurse (LPN) provides direct nursing care to the residents, and supervises the day‑to‑day nursing activities performed by care staff. Schedule Full‑time (at least 30 hours per week) 7:00 am–3:00 pm; also Part‑Time or Per‑Diem options available. Salary Range $38.00 – $41.00 per hour. Responsibilities and Duties Supervise care staff in accordance with current state regulations and community policies. Monitor the health, safety, and well‑being of all residents. Assist in training and monitoring of medication administration. Maintain clinical quality assurance in accordance with federal, state, and local standards. Promote the highest degree of service to our residents while leading and demonstrating the mission of the company. Conduct thorough resident assessments. Qualifications Graduation from an accredited School of Nursing with current LPN license. At least one year of experience in long‑term care, assisted living, home health, or hospital setting preferred. Experience working with residents with Alzheimer’s or other related dementias. Ability to handle multiple tasks. Knowledge of federal and state regulations and nursing practices applied to health and wellness residents. Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel); ability to learn new applications. Ability to perform physical activities required, such as standing, walking, sitting, using fine and gross motor skills, reaching, balancing, stooping, kneeling, crouching, speaking, hearing, and smelling.
