OTHER QUALIFICATIONS:

Successful completion of Background, MVR and DFWP Screenings. Each FACT IL team member contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness.

SUMMARY

The FACT IL Program is a multi-disciplinary, clinical team which assumes responsibility for directly providing needed treatment, rehabilitation and support services to adult persons with severe and persistent mental illness promoting recovery, empowerment, self-determination and decision‑making for each FACT IL participant.

EDUCATION/EXPERIENCE

Bachelor's degree in a behavioral science field, with preference for individuals who have completed a Targeted Case Management Certification (TCM). Previous experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required. ESSENTIAL DUTIES AND RESPONSIBILITIES

Working knowledge of Microsoft Office Programs, email, and the use of theinternet is required. With the client, translate assessment outcomes into functional domains, prioritize needs under each domain into immediate, short‑term, and long‑term categories, and identify appropriate case management interventions. Conducts mental health status assessments and may assist with biopsychosocial assessments as needed. Involve meaningful collateral sources/others in initial and on‑going case management service planning as appropriate and required. Use ongoing assessment and collaboration with the client and meaningful collateral sources/others to review and update the case management plan to reflect emerging needs and maintain relevance. Develop professional networks in order to identify and maintain information about available community resources to meet client needs. Make formal and informal referrals to service providers identified in the case management plan. Provides treatment, rehabilitation, and support services in accordance with DCF Guidance 36 Linking, Advocating, Transitioning, & Recovery Support (LATRS) Florida Assertive Community Treatment (FACT), Tier 2. Assures the accuracy, completeness and confidentiality of clinical records. Advocates for individuals served by the program to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and monitors and evaluates the effectiveness of the service and the person’s satisfaction. Provides coordination and linkage to community‑based services to improve stability using the social determinants of health. Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills. Provides education related to symptoms, medications, and mental health issues. Provides service coordination for an assigned group of individuals which includes monitoring process made towards goals and objective and developing, writing, implementing, evaluating and revising the recovery plan in partnership with the individual and other providers.

KEY PERFORMANCE INDICATORS

Documentation Timeliness- Complete all service data entry billing and documentation within 24 hours. (Assessments, Trackers, Assessment tools such as ASAM, GPRA, DLA-20, Outreach notes, Care Coordination notes, Miscellaneous notes, Individual notes, Group notes, Psych notes, Med Admin notes, Targeted Case Management Assessments, Targeted Case Management Service Plans, Targeted Case Management notes, etc.) in accordance with policy. 100% of the required training will be completed within specified time frames. 99% of events provided will be on the active service plan. Will have 99% accuracy in E.H.R. on entering information. Achieve 1350 annual impact hours per fiscal year (30 weekly hours) in accordance with Policy HR-56 Impact Standards for Case Specialist. #J-18808-Ljbffr StartingPoint
