Registered Nurse (RN) – Hospice Admissions

Location: Denton County, Texas
Job Type: Full-Time
Salary: $85,000–$95,000 per year

Additional Compensation

• Mileage reimbursement: $0.65 per mile
• Cell phone reimbursement: $60/month

Job Overview

We are seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to join our team in Denton County, TX . As the first clinical point of contact, you will guide patients and families through the hospice admission process with empathy, professionalism, and expert clinical care. This position is ideal for an RN who is passionate about hospice philosophy and providing exceptional end-of-life support.

Responsibilities

• Conduct comprehensive patient assessments to determine hospice eligibility.
• Guide patients and families through the hospice admission process with compassion and professionalism.
• Develop individualized plans of care for newly admitted patients.
• Collaborate with the interdisciplinary care team to ensure smooth patient transitions.
• Provide skilled nursing care based on individualized care plans.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in an on-call rotation as required.
• Perform additional clinical and administrative duties as assigned.

Qualifications

• Current, active Texas Registered Nurse (RN) License .
• Minimum 1 year of Hospice or Senior Care experience required.
• Strong understanding of hospice philosophy, hospice eligibility, and end-of-life care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage daily responsibilities effectively.
• Flexible schedule with the ability to adapt to changing patient needs.
• Valid driver's license, reliable transportation, and current auto insurance.
• Must be at least 21 years of age .
• Successful background check and professional references required.

Schedule

• Full-Time
• Monday–Friday, 8:00 AM–5:00 PM
• On-call rotation required
• Travel throughout assigned territory
