Lead Care Manager - ECM Field-Based (South Bay)

BLEHEALTH seeks a Lead Care Manager to coordinate care for chronically ill and high-risk members in the South Bay region. The role involves working with members, families, providers, hospitals, and community resources to deliver Enhanced Care Management with a person-centered approach.
