Job Summary
Outpatient Physical Therapist located in Corydon, Indiana. This position involves evaluating patients, developing individualized treatment plans, and performing patient care in an outpatient setting. Candidates are required to bring their valid physical therapy license on the first day of work.

Job Details

• Schedule: Days shift, 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 0+ years of outpatient clinical experience; CPR Certification; active/applicable license for Indiana; and successful completion of an assignment with the agency or organization previously is highly desired.
• Additional Information: Evaluates all assigned patients and develops, implements, and modifies treatment plans based on patient needs and physician directives. Responsible for the delegation and direction of therapy services. Completes medical record documentation accurately and timely, following organization policies and regulatory guidelines. Participates in student programs providing supervision and educational support. Maintains professional licensure and pursues ongoing professional development. Adheres to organizational standards for ethical practice, safety, and performance improvement initiatives. Demonstrates skills necessary to perform job functions in accordance with patient care populations served.
